What should I bring to my intake appointment?

Please bring the following items:

  • Photo ID (passport, driver’s license, state ID, birth certificate or other form of identification) 
  • Social security number
  • Health insurance card 
  • Additional documents may be recommended if you require financial assistance

What happens during my first appointment?

Your first visit is usually an intake or assessment appointment. This means you will be meeting with licensed mental health professional who will ask you questions regarding your history, current concerns, and goals for treatment. This provides us with background information to create the best course of care. 

How long will the appointment last?

The first appointment usually lasts between 60 to 90 minutes, depending on your needs. 

Will I start therapy immediately?

Not always. The initial appointment primarily focuses on getting to know you. You will have what is known as a hand-off; this is an introduction to your care team who will be directly involved in your care. 

Is everything I share confidential?

Yes. The information you share is confidential and protected under the Health Insurance Portability and Accountability Act (HIPAA). In certain situations, such as when there is a risk of harm to you or others, information may need to be shared. This is explained during your initial assessment.

What if I don’t know what to say or I’m scared?

That is normal and understandable. Taking the first step in treatment can be tough but the hardest part is asking for help. There is no wrong answer and you do not have to divulge everything in the first meeting.

Can I bring someone with me? What if I need a translator?

Patients may bring a support person to their appointment. A signed Release of Information form is required to authorize communication with that individual. Translation services are available.

How do I pay for the appointment?

You will speak with a patient financial advocate before your appointment if you do not have insurance coverage. You may be eligible for financial assistance.

What should I expect after the first appointment?

After your intake appointment, you will be assigned to a care team which includes a case manager, psychiatrist and other providers based on your needs.
