Healthy Transitions/ACT Young Adults is a program designed to assist youth and young adults ages 16 to 25 who are experiencing first episode psychosis (or FEP) and other significant mental health disorders. Tailored to their specific goals, our services are designed to help them gain independence in self-managing their illness.

Healthy Transitions provides intensive, multidisciplinary services using evidence-based treatment models such as cognitive behavior therapy, coordinated specialty care and the wraparound approach that focus on engaging the youth/young adult in learning strategies to manage their symptoms during early stages of their illness and helping the family support their emotional and social challenges during the transition to adulthood.
