Community Support Services

Gateway Healthcare’s community support programs provide coordinated care for adults 18 and older with qualifying mental health conditions and a higher level of need. The programs are designed to treat the whole person by coordinating primary care, specialist care, and behavioral health services. Our mission is to help people who have the following diagnoses reach a level of recovery that enables them to lead productive, independent lives: 

  • Schizophrenia
  • Psychotic disorders
  • Schizoaffective disorder
  • Bipolar disorder
  • Recurrent major depressive disorder
  • Obsessive-compulsive disorder
  • Borderline personality disorder
  • Other disorders that significantly limit a person’s independent functioning

A Team You Can Count On

Our community support programs deliver services primarily in the home and community. Each client is assigned a primary staff member who coordinates their ongoing treatment. The treatment team includes:

  • Case managers
  • Peer support professionals
  • Vocational specialists
  • Registered nurses
  • Therapists
  • Psychiatrists

A Wide Range of Strength-Based Therapeutic Services

We conduct a thorough diagnostic and functional assessment to determine the individual’s acuity level and program needs. Our person-centered treatment planning process establishes the roadmap for the focus of services and supports that will be provided. 

Our Programs

Services for individuals with complex and serious mental illness focus on helping them become independent and integrated into the community as they experience recovery. They require a multidisciplinary team approach with core services delivered primarily in the home and community and include clinical treatment, crisis intervention, psychosocial rehabilitation, case management services, integrated health home services, and care coordination.

Gateway delivers these services through the evidence-based practice model called Assertive Community Treatment (ACT). Services can vary in intensity depending on the needs of the client and the stage of their recovery. Many clients struggle with stable housing and employment, chronic illnesses, and substance use issues. The ACT treatment model tailors the multidisciplinary treatment to the client needs, reducing hospitalizations and improving quality of life for people living with severe mental illness.
