Medical Records at Saint Anne's Hospital

We are committed to keeping your protected health information (PHI) private, and we are required by law to respect your confidentiality. If you need copies or access to medical records, you can:

  • Log into our MyChart patient portal to access your records.
  • Fill out the Release of Information (ROI) form below and fax it to our office or bring it to us in person.
  • Call our Medical Records Department to request that record be prepared for you. You will be required to pick it up in person with a valid driver's license.

Phone numbers and request forms are available below on this page. We cannot fax or e-mail medical records without written consent from the patient (this must be done in-person at the time of your visit and varies by department). We also cannot release information from your medical records over the phone.

When you send a request to receive your medical records by fax, please be sure to provide a valid government-issued picture ID such as a driver’s license, identification card, passport, or comparison of signatures documented in the medical records.

If someone other than the patient is requesting to fax or mail the records, that person needs to provide a government issued picture ID and an original signed authorization letter from the patient. There may be a charge for creating a copy of your records.
