Both physicians who practice in this office are transitioning to other roles.

Why Is My Provider Leaving and Where Are They Going?

Your provider has decided to transition into another role at a new concierge medicine practice.

When Will I Know Who My New Clinician Will Be and What My New Practice Will Be Going Forward? 

A formal letter has been mailed to you with detailed instructions about the next steps in your care based on your response to our previous communication.

Due to the provider shortage in our region and across the country, any continued care with us will be at a location outside of the South County area. Because of the high volume of patient transfers, requests for a specific provider or location cannot be guaranteed. We appreciate your patience and understanding as we work to ensure your continued care.

Can I Choose the Provider and Practice I’m Assigned To?

We understand how important it is to feel comfortable with your provider. While we try to honor requests for specific providers or locations whenever possible, assignments are based on location and provider availability, and requests can’t be guaranteed.

Why Can’t I Continue My Care at Another Brown Health Primary Care Practice within the South County Area?

We are experiencing a national and regional primary care provider shortage, limiting our availability. Once your provider leaves, our currently available clinicians will be located at our practices outside of the South County area.

What If I Don't Respond by the Deadline in the Letter I Received?

The deadline has passed to opt in using the link provided in the initial letter you received about your provider leaving. If you did not respond to that letter as indicated, we sent a final follow-up letter offering you the opportunity to let us know if you’d like to establish care with another Brown Health Primary Care clinician outside of the South County area.

Contacting us after the date specified in that letter may result in a delay in being assigned to a new clinician. That message will also include a form to transfer your care to another primary care provider, should you choose to receive care elsewhere.
