These programs advance health equity by addressing social determinants of health through social needs screening, navigation support, workforce initiatives, and advocacy in the criminal legal system.
Connect for Health (C4H)
This program screens patients for health-related social needs and provides navigation support to secure basic needs like food, heating assistance, clothing, and more. In doing so, C4H bridges medical, community and social services to help patients achieve improved health and wellbeing. C4H not only provides medical treatment when patients are sick, but also addresses the social drivers of poor health. Connect for Health operates at Rhode Island Hospital’s Center for Primary Care and Hasbro Children’s’s Pediatrics Primary Care, Adolescent Health Care and Medicine Pediatrics clinics.
